Extraction — when it really is the right answer
Modern restorative dentistry can save teeth that even a generation ago would have been removed without much thought. Root canals are more comfortable and more predictable than they used to be. Crowns, inlays, and onlays can rebuild a heavily damaged tooth. Implant-supported restorations can replace a single missing tooth without grinding down healthy neighbors. Given these options, our default philosophy at Sunset Smiles is always to save the tooth when reasonable. We extract only when extraction genuinely is the best long-term answer — not as a convenience or a quick fix.
That said, there are situations where extraction is the correct call. Severe decay that has destroyed too much of the tooth to be restored. A vertical root fracture that no amount of crown or root canal can fix. A persistent or recurring infection that does not respond to endodontic treatment. Orthodontic crowding where a tooth needs to be removed to make room for proper alignment. And of course, impacted or partially erupted wisdom teeth that are damaging neighbors or repeatedly becoming infected. In each of these cases, doing the extraction promptly often prevents bigger problems later.
How a gentle extraction actually goes
The most important step in any extraction is thorough numbing. Dr. Martinez takes the time to ensure the area is completely numb before any procedure begins — most patients describe modern extractions as a strange experience of pressure and movement, but no pain. For more complex cases (impacted teeth, surgical extractions requiring a small incision), we use enhanced techniques and may consider light sedation; we will discuss what is appropriate for your specific situation during the evaluation visit.
After the tooth is removed, we focus on aftercare and replacement planning. Modern technique uses bone-preserving methods — particularly important if you are planning to replace the tooth with an implant later. A small bone graft placed in the socket at the time of extraction, called socket preservation, can keep the jawbone intact and make eventual implant placement easier and more predictable. We discuss this at the evaluation, not as a sales pitch, but because the decision has long-term consequences.
Replacement planning matters
A missing tooth changes the mouth gradually over time — neighboring teeth begin to drift, the opposing tooth in the other arch can start to over-erupt, and the underlying jawbone slowly resorbs without the chewing force of the missing tooth to stimulate it. None of this is dramatic in the short term, but over years it can complicate eventual replacement and affect your bite. For this reason, we discuss replacement at the extraction visit itself — whether that means an implant, a bridge, or a partial denture, or simply healing and observation if the tooth was a wisdom tooth or non-functional molar that does not need to be replaced.
For simple extractions, most patients return to normal eating within three to five days. Surgical extractions take slightly longer. Bone healing in the socket takes six to eight weeks, which is also when implant placement is typically planned if that is your replacement choice. We follow up after every extraction to make sure healing is on track, and we are reachable by phone if anything feels unusual. Pain after extraction is typically mild and managed with over-the-counter pain relievers; significant pain is uncommon and we want to hear about it if it occurs.

Will the extraction hurt?
The procedure itself shouldn't hurt — modern anesthesia is very effective and Dr. Martinez takes the time to ensure you're fully numb before starting. Soreness afterward is normal and usually manageable with over-the-counter pain relievers for a day or two.
How long does it take to heal?
The socket itself starts closing within the first week. Most patients return to normal eating within 3-5 days for simple extractions, slightly longer for surgical extractions. Full bone healing takes 6-8 weeks, which is also when we typically plan implant placement if you're replacing the tooth.
Do I need to replace the tooth right away?
Not immediately — but the longer a gap stays empty, the more neighboring teeth shift and the more jawbone is lost. We discuss replacement options (implant, bridge, or partial denture) at the extraction visit so you can plan.
Will I need a bone graft?
Sometimes. If you're planning to replace the tooth with an implant, a small bone graft at the time of extraction (called socket preservation) can keep the jawbone intact and make implant placement easier later. We'll discuss whether it's needed in your case.
What about wisdom teeth?
We extract many wisdom teeth here, particularly when they're erupted or partially erupted. For fully impacted wisdom teeth or complex cases, we may refer to an oral surgeon — but the consultation and planning happens with us.
