Periodontal assessment
We measure the pocket depth around each tooth, examine the gum tissue, and review X-rays to understand exactly where you are — from gingivitis (early, reversible) to periodontitis (advanced, manageable).
Scaling & root planing
If a deep cleaning is needed, Dr. Martinez and our hygienist gently remove tartar and bacteria from below the gumline and smooth the root surfaces so the gums can re-attach. We typically numb the area for comfort.
Targeted antimicrobial therapy
For specific deep pockets, we may place a localized antibiotic that releases gradually to support healing. This isn't always necessary — only when clinically indicated.
Periodontal maintenance
Once initial treatment is complete, you'll move to a periodontal maintenance schedule — typically every 3 to 4 months instead of 6 — to keep the disease from coming back.

Understanding gum disease
Periodontal disease is the leading cause of tooth loss in adults, and what makes it particularly insidious is how quietly it progresses. Unlike a cavity that aches or a chipped tooth you can feel, gum disease often produces no pain at all until the damage is already substantial. By the time bone loss has progressed enough to cause loose teeth, the disease has typically been present for years. This is why professional periodontal evaluation — measuring the pocket depth around every tooth, examining gum tissue color and tone, reviewing X-rays for bone level — is the only reliable way to catch the disease at a stage where it can still be fully controlled.
The disease itself is caused by bacterial plaque that hardens into tartar (calculus) and migrates below the gumline. The body's immune response to that bacteria — chronic inflammation — is what actually damages the supporting structures around the teeth. Over time, this inflammation breaks down the periodontal ligament that holds teeth in place and the bone underneath. The good news is that the process responds well to treatment, especially when caught early. At the gingivitis stage, the disease is fully reversible. Even at the periodontitis stage, where bone loss has begun, the disease can typically be controlled and stabilized so patients keep their teeth for life.
Scaling and root planing — the standard initial treatment
The initial treatment for active periodontal disease is a deep cleaning called scaling and root planing. Performed by Dr. Martinez and our hygienist with the area typically numbed for comfort, the procedure removes the tartar and bacterial film that have accumulated below the gumline. The exposed root surfaces are then smoothed (planed) so that the gum tissue can re-attach more cleanly and so that bacteria have less rough surface to colonize in the future. The work is generally done in two visits, treating one side of the mouth per session, to keep each appointment short and comfortable.
Patients are often surprised how comfortable deep cleanings actually are. The historical reputation comes from older techniques performed without adequate anesthesia. With modern numbing protocols and unhurried technique, most patients tell us the experience was much less intimidating than they had imagined. Some mild gum tenderness for a day or two afterward is normal, and easily managed.
Periodontal maintenance — the long-term plan
Once initial therapy is complete, patients move to a periodontal maintenance schedule — typically every three to four months instead of the usual six. This more frequent rhythm is essential because periodontal bacteria recolonize the deep pockets that have formed. Maintenance visits keep the bacteria below the threshold at which they cause further damage, and they catch any new active disease early enough to address quickly. Patients who stay on their maintenance schedule almost always keep their teeth long-term; patients who drift back to six-month cleanings often see the disease progress again.

In its earliest stage — gingivitis — yes, fully reversible with good professional cleaning and home care. Once it progresses to periodontitis (bone loss has begun), it's not reversible, but it can be controlled and stabilized with treatment so you keep your teeth.
What does a deep cleaning feel like?
More involved than a regular cleaning, but not painful. We typically numb the area being treated. Most patients are surprised by how comfortable it is. We work in quadrants over one to two visits depending on how much treatment is needed.
How often do I need periodontal maintenance after treatment?
Most periodontal patients move to a 3- or 4-month maintenance schedule (instead of the standard 6 months). This more frequent rhythm keeps bacteria from re-colonizing the deep pockets that have formed.
Does insurance cover deep cleanings?

Can I just get a regular cleaning instead?
