Modern fillings — what changed
If your dental memories include silver amalgam fillings, mercury-vapor concerns, and overly aggressive drilling, you'll find modern restorative dentistry to be a different experience entirely. Today's standard filling material is tooth-colored composite resin — a durable polymer matrix reinforced with ceramic and glass particles, bonded directly to the tooth structure with adhesive chemistry. The result is invisible, mercury-free, and structurally integrated with the tooth in a way that older amalgam fillings never could be.
The shift to composite was driven by several advantages that compound over time. Because composite bonds directly to the tooth, the cavity preparation can be more conservative — Dr. Martinez removes only the decayed tooth structure, rather than carving an undercut shape required to mechanically retain a silver filling. More natural tooth structure means a stronger, longer-lasting tooth. The bonding chemistry also creates a tight seal against bacteria, reducing the risk of new decay developing around the edges of the restoration. When fillings are the right answer
A filling is the right treatment for moderate amounts of tooth decay that have not yet reached the nerve and have not destroyed too much of the tooth's structure. For very small early lesions, we may recommend watchful waiting and improved home care — not every visible spot on an X-ray needs immediate treatment, and being conservative often saves patients money and tooth structure. For larger areas of decay where a filling would leave too little tooth behind to support normal chewing forces, we typically recommend an inlay, onlay, or crown instead. Picking the right restoration for the right tooth is part of what good restorative dentistry looks like.
Replacing old silver fillings is a frequent question. The honest answer is: it depends. If a silver filling is intact, sealed at the margins, and not causing problems, simply monitoring it at your routine cleanings is usually appropriate. Replacement becomes a good idea when the silver filling shows cracks, is leaking, has decay underneath, has caused the tooth to develop fracture lines, or when the patient simply wants an aesthetic upgrade. We will share what we see and what we would recommend if it were our own mouth — and we will not push replacement of fillings that are doing their job.
How the procedure actually feels
Most patients are surprised by how comfortable modern fillings are. With careful numbing technique, the procedure itself is essentially sensation-free — you may feel pressure or vibration, but not pain. We use small, precise instruments, take frequent comfort breaks if needed, and explain each step as we work. For patients who experience dental anxiety, simply telling us beforehand changes our pace; we have plenty of options to make the visit easier, from headphones to extra time to anti-anxiety considerations.
After the numbing wears off, mild temperature sensitivity for a few days is normal and resolves on its own. Significant or lingering pain after a filling is uncommon and we want to know about it — sometimes a high spot in the bite needs to be adjusted, which takes about five minutes. Composite fillings placed properly typically last seven to fifteen years, often longer in low-stress areas of the mouth. When eventually they do wear out or chip, replacing them is a straightforward procedure that we plan into your routine cleanings.
