When you complete a form on our Site (appointment request, contact form, insurance verification, newsletter signup), we collect:

3. SMS / text message consent (TCPA)

By submitting a form on this Site that includes your phone number, you expressly consent to receive SMS / text messages from Sunset Smiles Cosmetic Dentistry related to your inquiry, appointment scheduling, appointment confirmations, and appointment reminders.

Consent to receive text messages is not a condition of receiving dental care. You may decline to provide your phone number, and we will contact you only by email.

Message frequency: You may receive up to four (4) messages per month per active inquiry or appointment. Message and data rates may apply.

To opt out: Reply STOP to any text message you receive from us at any time. We will confirm your opt-out and will not send you further marketing or scheduling text messages. You may continue to receive transactional messages related to existing appointments unless you contact us directly to terminate.

4. How we share information

We share information only with:

  • Service providers who help us operate the Site and our practice — including our website host (Netlify), form provider (Netlify Forms), email service, analytics provider (Google Analytics), advertising platform (Google Ads), scheduling software (Dental4.me), and automation tools (Make.com). These providers are contractually limited to using information only to perform services for us.
  • Your insurance carrier, only when you submit an insurance verification request, and only to confirm your eligibility and benefits.
  • Legal authorities when required by law, subpoena, or court order, or to protect our legal rights and the safety of patients and staff.
  • A successor entity in connection with a sale, merger, or transfer of the practice. If this occurs, we will post a notice on this Site and notify you by email if we have your address.

5. Cookies and tracking technologies

We use the following tracking technologies on our Site:

  • Google Analytics 4 (GA4) — measures Site usage in aggregate (pages viewed, clicks, scroll depth, traffic source). GA4 uses cookies and processes IP addresses. We use IP anonymization where applicable.
  • Google Ads conversion tracking — measures whether visitors who clicked an ad later submitted a form or called us. Used only to evaluate ad performance.
  • Strictly necessary cookies — used to remember your accessibility preferences (high contrast, reduced motion, etc.) and to operate the Site.

You can opt out of analytics tracking by:

  • Installing the Google Analytics Opt-Out Browser Add-on

6. HIPAA and protected health information

Information about your dental treatment, diagnosis, medical history, and clinical records is protected health information ("PHI") under HIPAA and is governed by our separate Notice of Privacy Practices, which we provide to every patient at the first appointment. Important: Email and standard web forms are not encrypted methods of transmission. Please do not send detailed medical or treatment information through this Site. For clinical questions, call us directly or wait until your appointment.

The general category you select on our appointment form (e.g., "cosmetic consultation," "dental emergency") is used solely to route your request to the appropriate team member and is handled with the same care as the rest of your contact information.

7. Your privacy rights

Depending on where you reside, you may have the following rights regarding personal information we collect through the Site:

  • Right to know what personal information we have collected about you
  • Right to access or request a copy of that information
  • Right to correct inaccurate information

  • Form submissions from non-patients — 24 months from last contact, then deleted.
  • Patient records (PHI) — retained per Florida dental record retention requirements (currently 4 years from date of last entry for adult patients; longer for minors) and HIPAA standards.
  • Analytics data — Google Analytics is configured to retain user-level data for 14 months, after which it is automatically deleted.
  • SMS opt-out records — retained indefinitely to honor your opt-out.

10. Children's privacy

Our Site is not directed to children under 13, and we do not knowingly collect personal information from children under 13 through the Site. If you are a parent or guardian booking an appointment for your child, please submit the request using your own contact information. If you believe a child has submitted information through our Site, contact us and we will delete it promptly.

11. Third-party links and services

Our Site contains links to third-party services we use or recommend, including our online booking system (Dental4.me), patient financing (Cherry), our membership plan platform (Clerri), Google Maps, and Google reviews. These services have their own privacy policies. We are not responsible for their privacy practices, and we encourage you to review their policies before providing information to them.

We may update this Privacy Policy from time to time. When we do, we will update the "Last Updated" date at the top of this page. For material changes, we will post a notice on the Site and, where required by law, notify you directly. Your continued use of the Site after the changes take effect constitutes acceptance of the updated policy.
